First Visit
Upon your initial phone call, our experienced team member will collect information from you to assist in scheduling the proper type of appointment . Your initial appointment will consist of a diagnostic, digital x-ray and diagnostic consultation. Our Doctor will explain your diagnosis and treatment options. Treatment may be done the same day as the consultation provided we have answered all your questions and treatment has been explained. If treatment with I.V. Sedation is desired, this consists of a I.V. consultation and separate appointment for the treatment, If insurance is being utilized we will contact the carrier and provide you with an estimated co-payment prior to your visit.Please ask our team member to assist in making the best appointment choice for your individual needs.
Please assist us by providing the following information at the time of your appointment:
- Your referral slip and any other information from your referring dentist.
- A list of medications you are presently taking.
- Any dental insurance information: including the insureds name, date of birth, employer, I.D. number or social security number (whichever the insurance company uses to identify the individual benefits) and the group number.
IMPORTANT: A parent or guardian must accompany all patients under 18 at all visits.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
